Exit Survey
	The purpose of this form is to gather information System-wide from faculty and staff who are leaving voluntarily. We would like to know your frank and candid feelings about a variety of aspects regarding your employment experience. This information will be used for administrative decision making, and presentations to the State Board of Higher Education, legislative committees, and commissions which address employee compensation and benefits.
